Guillermo Bernal comenzó su carrera como profesor de psicología en la Universidad de California, San Francisco (UCSF) en 1978. Trabajó en el Departamento de Psiquiatría en el Hospital General de San Francisco (SFGH), el hospital de la red de seguridad social para la gente de San Francisco. Ubicado en el Mission District, el barrio latino, el hospital atendía a los más necesitados, incluyendo a un gran número de residentes hispanohablantes de la ciudad.
